Actually, in Texas (and maybe some other parts of the South) the term "ma'am" is a term of respect that's used regardless how old the lady is. Pretty much anyone out of college is a ma'am to me.

out of respect i have always called man "sir" and women "ma'am" ...just the way i was brought up...

It's pronounced as it's spelled. The apostrophe is because it's a contraction of the word 'madam.' Yes, we still refer to women as madams in the South.

I don't think I've ever heard anyone say it. It's always just been"mam". It's seems like so much effort to pronounce the "a" twice.
