Below the Mason-Dixon line it's a cold sub style sandwich. Above the Mason-Dixon line, it's sub sandwich that they toast in a pizza oven after it's made. Here they're called hoagies.

Regional it is. When I go into a local pizza place, I order an Italian Grinder. The response is usually "toasted or cold". If i'm in Subway or a place like that, i'll call it a sub.

Bubbler is another regional term. Everyone else knows it as a water fountain.
