Make sure you are careful not to damage the MAF sensor, and make sure it's dry before reinstalling Inspect all vacuum and PCV hoses, replace if necessary Check for a dirty fuel filter and proper fuel pressure

almost sounds like a problem I had after cleaning throttle body , MAF sensor testing each injector Wirth battery suringe test & still nothing turned out to be the fuel pump pickup screen was clogged up with all the crap that comes with our gas.
I waited until I got low fuel disconnected fuel relay then removed my fuel pump cleaned it up & walk a runs sweet even gained better miles or here about 100 kilometers per tank.

and just this June July 2014 I flushed my fuel tank out yet as you can see how much came back , in with cleaning the throttle body I got an oil seperator and that alone was a huge help in keeping oil out of the intake.

But keep in mind it is an error code. It does not mean that it is really running lean Meaning a O2 Sensor could be off or another sensor giving the computer an incorrect reading indicating it is running lean. I have seen this many times and give an error of Bank# lean. Have you connected a analyzer and looked at the live data?
CD I have seen one bad tank of gas take out an O2 Sensor. Wasn't the First Trackey blowing them out too? Also I think I have read that putting on a CAI with out the reducer will give the same code. Computers Go to Love Them!!
I just read an article on top fuel and was surprised they didn't use O2's on the headers, but rather an 'exhaust gas temperature' sensor. If its not burning hot and equal, they know something is off.
