It's certainly possible a wheel weight came off, but I'd expect an out of balance wheel to give you move of a shimmy, as oposed to shaking the whole car. I'd say do a quick check to see that the lug nuts are tight, but you already took it to the dealer.

It depends upon how big the pothole was, LOL.

Edit: Just realized I meant to say "bent rim" earlier (causing wheel out of balance).

Driveshaft lose a balance weight? I had this happen on a 87' Mercury Cougar when it was like 3 months old. Very similar vibration.

Speed sounds to low for a wheel out of balance - not that it couldn't happen, but it seems to me unless its really out of balance, an imbalnced wheel wouldn't show up till the 50-60 mph range. - I'm going with Mach here (assuming nothing is damaged on the car).

The DS had one bolt come completely out and the other three were lose. It is a Powerhouse DS installed by Evolution a little over a year ago. The dealer said they should have used locktite... The sad thing is, this is going to cost me a couple hundred in labor to have it fixed.

You should try to get Evolution to reimburse you since its obviously there fault. You might remind them that because you had the fore site to stop driving it and took it to the shop you avoided what would have been a CATASTROPHIC failure resulting in tons of damage and possible injury and/or death.

I called, they did not seem to care to much... They gave me the number to Powerhouse and sent me on my way. Fred did ask how my car was running, which by the way, is only OK. Nothing great. It's been over a year since the part was installed so I doubt I have a leg to stand on if I really wanted to be reimbursed. I don't want to say anything bad about Evo, but I doubt I ever go back.
