code 41 (lean) on '92 LX5.0

I bought a mint '92 LX 5.0 which was stock except for long-tube headers, off-road H-pipe and Flowmasters. It had a few codes when I bought and have fixed all of them except for code 41 continuous memory code which is either "Heated O2 sensor out of range/always lean" or "no O2 sensor switching detected." They are both listed as 41 CM codes. After I bought it and read the codes I put on a catted x-pipe, 65mm TB/EGR spacer/TPS, new EGR valve and IAC motor. I cleaned the factory items when I bought it due to the throttle stickiing badly. It still stuck so I changed the TB and it's been fine. Also, I figured a little less airflow and a little more fuel would richen it up a little. Before and after the changes it runs great, no missing or smoking. The idle hunts a little bit between about 950-1100 rpm. The check engine light was on a lot when I first bought it (12-26-12), and now only comes on after warm up. Approx 20-25 minutes of driving. It will stay on until I shut it off. And then I can drive another 20-25 minutes before it comes on again. My next thoughts are possibly a "weak" injector/injectors. Or maybe new O2 sensors. I'm not a fan of just replacing stuff *****-nilly, without having an actual reason. Any thoughts on what I might check next or anything I might be missing? BTW my code reader manual says not to do the Ignition Timing Check or KOER test until I get a clean KOEO test, that's why I am going about this in a methodical way. Thanks.

Thanks Skunk, that's exactly the kind of info I was looking for. Looks a little complicated but my best friend is an electrician so I'm sure he could help me with some of the diag. Thanks again.
