A few years ago I accidentally dropped a heavy box of Christmas decorations on the crease by the front hood. It seemed to be in a horrible location and I was &*#! 
I called the local dealerships to see who they used. I called him and then drove two hours to his place. He fixed it while I waited (about 30 minutes), and $200 later I was on my way. I can not see the prior dent no matter how hard I try under all types of lighting.
The key is to find the right guy which is why I asked the dealers who they use. In this case, they were spot on.

Mine was done under my insurance coverage, so I don't remember the cost exactly. Seems like it was around $250. I have probably half dozen hail dents fixed. Like other said, I couldn't tell they were ever there when the tech finished. I would have paid that even if the insurance wouldn't.
Glenn, if you can bear to look at the damage for a while, parking the car out in the sunshine might result in Mama Nature popping a couple of those dents out and/or shrinking a few. If you are paying out-of-pocket, this could soften the hit a little. Just a thought.
I have 6 small dents on my roof of the Raptor from hail acouple weeks ago and was quoted $200 - $300 depending on how much of a pain the headliner is to drop sense I have a sunroof.
I do PDR and yes the prices vary based on the size and location of the dent. Every dent is different, so it's hard to have a set price for every dent. Also, today's cars are a mix of aluminum and steel panels. Aluminum is much more difficult to work with so an equal size dent to one on a steel panel, will cost more. Also, I don't drill, so a dent in a tough spot may require more panel removal or glue pulling.
Dry ice and heat don't "remove" dents. The heat causes the panel to swell and moves the metal around a may cause certain dents to "pop" or shrink, but the dent is still there. If anybody ever suggests heating the panel then hitting it with dry ice, run away! That shocks the metal and can cause the paint to separate from the panel.
