Warped rotor

So I put 1" spacers in the rear of my 95 cobra...the lugs didnt let the wheel sit right on the driver rear and now it resulted in a warped rotor...just has a constant squeak. I did full brakes about 5k miles ago. Would I be safe just to change the one or do I have to change both the rears? And I'm assuming pads don't have to be changed.

The lugs didn't let the wheel sit right on the DS rear rotor. What cause the issue? Wrong type of lug?

Also no it shouldn't be a problem to just change one rotor, would be best to swap out the pads as well but you'll want to scuff the pad to eliminate glazing and level the surface.
